تعادل ایستگاهی در آمپوته های اندام تحتانی ؛ مروری بر مقالات

<p style="text-align: justify;"><strong>زمینه و هدف:</strong> قطع عضو های اندام تحتانی معمولا به علت ضربه،بیماری های عروقی،دیابت و یا اختلالات مادرزادی ایجاد می شود.افراد دچار قطع عضو، توانایی ایستادن و راه رفتن را با توجه به سطح قطع عضو خود از دست می دهند.تاثیر سطح قطع عضو،نوع قطع عضو و یا علت قطع عضو بر اختلالات بالانس به طور شفاف مشخص نشده است.علاوه بر آن تاثیر قطعات پروتز بر تعادل افراد حین ایستادن مشخص نیست.بنابراین هدف از این مقاله مروری پیداکردن رابطه بین میان فاکتورهای ذکر شده در بالا و اختلالات تعادلی با توجه به تحقیقات موجود می باشد.</p>
<p style="text-align: justify;"><strong>روش:</strong> یک جستجوی الکترونیک در پایگاههای اطلاعاتی PubMed، ISI web of science و Embase از سال 1960 الی 2012 انجام گردید.کیفیت مقالات با استفاده از ابزار Black and Down مورد ارزیابی قرار گرفت.کلمات کلیدی زیر برای جستجو انتخاب گردید : Stability, Prosthesis, Standing, Balance, Prosthesis components</p>
<p style="text-align: justify;"><strong>نتایج: </strong>بر اساس کلمات کلیدی استفاده شده،100 مقاله پیدا شد که 20 مقاله با توجه به معیارهای انتخاب پذیرفته شدند.امتیاز گزارش دهی،روایی خارجی،روایی داخلی (بایاس)،روایی داخلی به ترتیب بین 4-9، 1-3 ، 3-5 ، 2-6 متفاوت بود.</p>
<p style="text-align: justify;"><strong>نتیجه گیری: </strong>نتایج این بررسی حاکی از آن است که تعادل ایستادن آمپوته ها به سطح قطع عضو،نوع قطع عضو و علت قطع عضو بستگی دارد.به علاوه خصوصیات پروتز مثل سفتی مچ پروتز بر تعادل داینامیک اثر گذار است. در حالی که تورشن آداپتور هیچ تاثیر مثبتی بر تعادل در حین راه رفتن و چرخش ندارد.توصیه می شود تاثیر سایر قطعات پروتز بر تعادل ایستگاهی سنجیده شود.</p>
<p><strong>Background:</strong> Lower limb amputations are caused by trauma, vascular disease, diabetes and congenital disorders. Amputees lose their ability to stand and walk based on the level of amputation. Contribution of the level of amputation, type of amputation, or cause of amputation to balance impairment has not been clearly defined. Furthermore, it is controversial whether prosthetic components influence standing stability or not. Therefore, the aim of this review article was to find the relationship between the above-mentioned factors and balance impairment in the available literature.</p>
<p><strong>Method:</strong> An electronic search was done via the PubMed, Embase and ISI web of knowledge data bases from 1960 to 2012. Quality of papers was assessed using the Down and Black tool.</p>
<p><strong>Results:</strong> Based on the used keywords, 100 papers were found of which 20 papers were selected in accordance with the selection criteria. The scores of reporting, external validity, internal validity (bias), internal validity (confounding) varied between 4-9, 1-3, 3-5, 2-6, respectively.</p>
<p><strong>Conclusion: </strong>The literature review confirmed that standing stability of amputees depends on level of amputation, type of amputation and cause of amputation. Moreover, prosthetic characteristics such as prosthetic ankle stiffness have influences on dynamic stability while torsional adaptor does not have any positive influence on stability during level walking and on turning stability. It is recommended to measure the influence of other prosthetic components on standing stability.</p>
